Child Health and Disability Program

The Child Health and Disability Prevention (CHDP) is a preventive healthcare program for low income children and youth in California.

CHDP provides periodic health assessments and services, care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services.  Health assessments are provided by enrolled private physicians, local health departments, community clinics, managed care plans, and some local school districts.

Eligibility Criteria

Eligibility Criteria

  • All Medi-Cal recipients from birth to age 21 are eligible for CHDP scheduled periodic health assessments and services based on the federally mandated Early and Periodic Screening, Diagnosis, and Treatment (EPSDT) Program
  • Children from birth to age 19 who are not eligible for Medi-Cal qualify for the CHDP schedule of periodic health services and assessments if their family income is equal to or less than 200% FPL guidelines.

  • Dental assessment
  • Nutritional evaluation
  • Developmental assessment
  • Review of health history
  • Height, weight and blood pressure measurements
  • Vision and hearing tests
  • Tests for anemia, blood lead poisoning and tuberculosis
  • Immunizations
  • Health Education
